L-Arginine is an amino acid that serves as a precursor to nitric oxide, a molecule involved in vasodilation and blood flow regulation. Its use in supporting or treating infertility is based on some scientific investigation, particularly regarding its potential effects on erectile dysfunction and sperm motility in men, as well as endometrial receptivity in women. Several small clinical trials have explored L-arginine supplementation for male infertility, showing modest improvements in sperm motility and count, likely due to enhanced blood flow and cellular function. In women, there is limited evidence suggesting possible benefits in cases of poor ovarian response or endometrial insufficiency, again likely tied to improved vascularization. However, the overall quality of evidence is low, with few large, well-controlled studies and inconsistent results. Most guidelines do not recommend L-arginine as a first-line treatment for infertility, and its use remains adjunctive rather than primary. While there is a plausible biological mechanism and some encouraging preliminary data, robust, large-scale randomized controlled trials are lacking.
