Ashwagandha (Withania somnifera) has a long history in traditional Ayurvedic medicine as a remedy for male and female infertility. Traditionally, it is considered a "rasayana" or rejuvenator, believed to enhance vitality, sexual health, and reproductive function. In recent years, scientific studies have begun to investigate these claims, particularly regarding male infertility.

Several clinical studies have shown that ashwagandha root extract may improve semen quality, sperm count, motility, and testosterone levels in men with infertility. For example, a 2010 study published in Fertility and Sterility found that ashwagandha supplementation significantly improved semen parameters and antioxidant levels in infertile men. Other research suggests that ashwagandha may help normalize hormonal imbalances and reduce oxidative stress, both of which can contribute to infertility. The adaptogenic properties of ashwagandha are thought to reduce stress-related hormonal disturbances, which can affect reproductive health.

However, the evidence is still considered moderate (rated 3/5) because most studies are small, have methodological limitations, or are focused primarily on male infertility. There is less robust evidence for its effects on female fertility, with only limited preliminary studies and animal data available. Overall, while traditional use is strong and early scientific results are promising, larger and more rigorous clinical trials are needed to fully validate ashwagandha’s efficacy in treating infertility.
