Cistanche tubulosa has a long history of use in traditional Chinese medicine (TCM) and other East Asian medical systems as a tonic herb, particularly for addressing reproductive health concerns including infertility, impotence, and weakness associated with the "Kidney Yang" deficiency pattern. The root of Cistanche, known as "Rou Cong Rong" in TCM, is believed to tonify kidney qi and essence, both considered foundational to reproductive capability in TCM theory. Historical texts and materia medica frequently recommend its use for both male and female fertility-related issues, such as low libido, erectile dysfunction, and female infertility, often as part of multi-herb formulas. Scientifically, the evidence supporting Cistanche tubulosa for infertility is limited, with most studies focusing on animal models. Several preclinical studies suggest that extracts of Cistanche can improve spermatogenesis, sperm motility, and hormone levels in rodents subjected to induced infertility or subfertility. Some studies attribute these effects to the presence of bioactive compounds such as echinacoside and acteoside, which may have antioxidant and hormone-modulating properties. However, well-designed human clinical trials are lacking, and the mechanisms remain unclear. Therefore, while traditional use is robust and preclinical data are promising, the overall evidence base remains weak, justifying a moderate rating.
