Thyroid hormone replacement is scientifically validated for treating certain forms of infertility, particularly in women with hypothyroidism. Hypothyroidism can disrupt menstrual cycles, ovulation, and endometrial receptivity, all of which are essential for conception and successful pregnancy. Numerous clinical studies have shown that correcting hypothyroidism with levothyroxine or other thyroid hormone preparations can restore normal menstrual function and improve fertility outcomes. The American Society for Reproductive Medicine and the American Thyroid Association both recommend screening for thyroid dysfunction in women with infertility and treating hypothyroidism to achieve a normal thyroid stimulating hormone (TSH) level before attempting conception.
However, there is no evidence that thyroid hormone therapy benefits women with normal thyroid function ("euthyroid") or that it should be used as a general fertility aid in the absence of documented deficiency. Its use is specifically indicated for individuals with diagnosed hypothyroidism or subclinical hypothyroidism, particularly when TSH is elevated. In summary, thyroid hormone therapy is an evidence-based treatment for infertility when thyroid dysfunction is present, but not otherwise.
