Tongkat ali (Eurycoma longifolia), a medicinal plant native to Southeast Asia, has a long tradition of use as an aphrodisiac and fertility enhancer in traditional medicine systems of Malaysia, Indonesia, and other nearby regions. In recent years, this traditional use has been investigated scientifically. Several clinical studies and systematic reviews suggest that tongkat ali supplementation can increase testosterone levels in men, improve sperm quality parameters (such as sperm concentration, motility, and morphology), and, in some cases, increase pregnancy rates among couples struggling with infertility. The mechanisms are thought to involve boosting endogenous testosterone production, improving stress hormone profiles, and acting as an antioxidant to protect sperm from oxidative damage. However, while these findings are promising, many studies are relatively small, short-term, or lack rigorous controls. Meta-analyses generally conclude that tongkat ali may be beneficial for male infertility, but larger, high-quality randomized controlled trials are still needed to confirm efficacy and safety. There is less evidence for effects on female infertility. Overall, tongkat ali’s use for male infertility is supported by both tradition and a moderate level of scientific evidence, though it should not replace standard medical interventions where indicated.
