Vitamin B12 is an essential water-soluble vitamin involved in DNA synthesis, red blood cell formation, and neurological function. Its use in supporting or treating infertility has some scientific basis, particularly in cases where deficiency is present. Vitamin B12 deficiency has been associated with infertility in both men and women. In women, low B12 levels can lead to ovulatory dysfunction, irregular menstruation, and improper development of the endometrium, which may hinder implantation. In men, B12 deficiency has been linked to impaired spermatogenesis, reduced sperm count, and motility issues.

However, supplementation with vitamin B12 appears to be beneficial primarily when there is a documented deficiency. For example, studies have shown that correcting B12 deficiency in women can restore ovulation and improve chances of conception. In men, some research suggests that B12 supplementation may improve sperm parameters in subfertile individuals, though results are mixed and most evidence comes from small studies or observational data.

There is little evidence to support vitamin B12 supplementation for infertility in people with normal B12 status. Major guidelines do not recommend routine B12 supplementation for infertility unless deficiency is suspected or confirmed. In summary, vitamin B12 is scientifically justified for treating infertility in the context of deficiency, but not as a general fertility enhancer.
