Anthocyanins are a class of flavonoid compounds found in various fruits and vegetables, particularly those with red, blue, or purple pigmentation (such as berries, red cabbage, and black rice). Their use in the context of Inflammatory Bowel Disorders (IBD), including Crohn's disease and ulcerative colitis, is supported by emerging scientific evidence, primarily from preclinical studies and a limited number of human trials. Anthocyanins have demonstrated anti-inflammatory, antioxidant, and gut barrier-protective effects in cell and animal models of IBD. These effects are thought to be mediated by the suppression of pro-inflammatory cytokine production (e.g., TNF-α, IL-6), inhibition of NF-κB signaling, reduction of oxidative stress, and modulation of the gut microbiota. A few small-scale human studies and pilot trials have shown that anthocyanin-rich extracts (from sources such as bilberry or blackcurrant) may reduce clinical symptoms and inflammation markers in patients with ulcerative colitis, but the quality and size of these studies are limited, and results are not always consistent. No large-scale, high-quality randomized controlled trials have definitively established their efficacy as a standalone or adjunct therapy for IBD. Thus, while the mechanistic rationale is strong and animal studies are promising, clinical evidence remains preliminary. Anthocyanins are generally considered safe as dietary components, but more rigorous research is needed to establish clinical guidelines for their use in IBD.
