Beta Hydroxybutyrate (BHB) is a ketone body produced during states of ketosis, such as fasting or ketogenic diets. Interest in BHB for inflammatory bowel disorders (IBD), such as Crohn’s disease and ulcerative colitis, is relatively recent and based primarily on preclinical studies. In animal models, BHB has demonstrated anti-inflammatory properties, including inhibition of the NLRP3 inflammasome, a key mediator of inflammatory responses implicated in IBD pathology (Youm et al., Nature Medicine, 2015). Additionally, ketone bodies like BHB may modulate gut microbiota and enhance the intestinal barrier, which are relevant mechanisms in IBD (Poff et al., Frontiers in Immunology, 2020). However, robust clinical evidence in human patients with IBD is lacking, and there are no established guidelines recommending BHB supplementation for IBD management. Most data come from mechanistic cell culture, animal models, and extrapolation from the effects of ketogenic diets. Thus, while there is scientific rationale and preliminary evidence for BHB’s anti-inflammatory effects, the clinical efficacy and safety of BHB supplementation in IBD patients remain to be established through high-quality clinical trials.
