Chymotrypsin, a proteolytic enzyme originally derived from the pancreas, has been used as a therapeutic agent for the management of injuries, particularly those involving soft tissue trauma, inflammation, and edema. The rationale for its use lies in its ability to break down proteins involved in inflammatory exudates and necrotic tissue, which is thought to accelerate the resolution of swelling, hematomas, and pain. Several clinical studies, primarily from the mid to late 20th century, have investigated the efficacy of chymotrypsin—often in combination with other enzymes such as trypsin and bromelain—in reducing postoperative swelling, sports injuries, and other inflammatory conditions. While there is a body of research suggesting modest benefits, including faster reduction of inflammation and improved healing times, the methodological quality of these studies varies, and more recent, large-scale randomized controlled trials are limited. Some meta-analyses and systematic reviews indicate that proteolytic enzymes may offer benefits over placebo for soft tissue injuries, but the effect size is generally small and there can be significant heterogeneity among studies. Regulatory authorities in some countries still approve chymotrypsin for this purpose, whereas in others its use is more limited or controversial. Overall, while chymotrypsin is not solely a product of tradition and does have some scientific support for use in injury management, the evidence is moderate and not universally accepted. Further high-quality studies are needed to clarify its efficacy and safety profile.
