Wintergreen (Gaultheria procumbens) oil, primarily composed of methyl salicylate, has been traditionally used to relieve pain associated with minor injuries, muscle aches, and joint discomfort. Its use is rooted in folk medicine practices among Native Americans and in Western herbal traditions, where wintergreen leaves or oil were applied topically to reduce pain and inflammation. Methyl salicylate, the active constituent, is a counterirritant that produces a sensation of warmth and increases local blood flow, which may help distract from underlying pain or injury.

While there is some biological plausibility due to the structural similarity of methyl salicylate to aspirin (acetylsalicylic acid), which is known to have anti-inflammatory and analgesic properties, rigorous clinical trials supporting the efficacy of topical wintergreen for injury recovery are lacking. Most modern topical analgesic formulations that include wintergreen are based on its traditional use and its counterirritant effects, rather than robust scientific validation. Adverse effects, particularly toxicity from excessive absorption, are possible, especially in children. Therefore, while tradition strongly supports its use, scientific evidence is limited, and it should be used with caution.
