Alkaloids have a long history of traditional use in the management of gastrointestinal disorders, including Irritable Bowel Syndrome (IBS). Among them, hyoscine (also known as scopolamine) and atropine—tropane alkaloids derived from plants such as Atropa belladonna—have been used as antispasmodics to alleviate abdominal pain and cramping associated with IBS. The traditional use of these alkaloids is based on their ability to relax smooth muscle in the gastrointestinal tract by antagonizing muscarinic acetylcholine receptors. This action reduces spasms and motility, providing symptomatic relief for some IBS patients.

Scientific studies have provided some support for the effectiveness of antispasmodic alkaloids in treating IBS, but the evidence is moderate rather than robust. Meta-analyses and systematic reviews suggest that antispasmodics, as a class (which include both alkaloid and non-alkaloid drugs), can reduce pain and improve other symptoms in IBS. However, the data specifically for plant-derived alkaloids like hyoscine are less extensive, and concerns regarding side effects (dry mouth, blurred vision, urinary retention) limit their widespread use. Overall, alkaloids for IBS are best characterized by their traditional use, with moderate evidence supporting their symptomatic benefit.
