Flax fiber, derived from the seeds of the flax plant (Linum usitatissimum), is a source of soluble and insoluble dietary fiber and has been studied for its effects on digestive health, including in patients with Irritable Bowel Syndrome (IBS). Several clinical studies and reviews have investigated the effect of flaxseed fiber on IBS symptoms, particularly constipation-predominant IBS (IBS-C). The evidence suggests that flax fiber may improve stool frequency and consistency due to its bulking and water-binding properties, and it may also modulate gut motility and the gut microbiome. For example, a randomized controlled trial published in 2011 found that ground flaxseed was superior to psyllium in improving symptoms of constipation and abdominal discomfort in IBS patients. However, some studies have shown mixed results, with a few reporting no significant difference compared to placebo. Systematic reviews generally conclude that while flax fiber may offer symptom relief for some IBS patients, particularly those with constipation, the overall quality of evidence is moderate, with a need for larger, well-designed trials. There is no evidence of harm from flax fiber when used appropriately, but patients with IBS-D (diarrhea-predominant) should use caution as it may exacerbate symptoms. In summary, flax fiber is used with some scientific support, especially for IBS-C, but is not a universally recommended treatment for all IBS cases.
