Lactobacillus salivarius is a probiotic bacterium that has been studied for its potential benefits in managing Irritable Bowel Syndrome (IBS). The rationale for its use is based on the growing understanding that alterations in the gut microbiota composition may contribute to IBS symptoms, such as abdominal pain, bloating, and irregular bowel habits. Probiotics, including L. salivarius, are thought to help restore a healthy balance of gut bacteria, modulate immune responses, and reduce inflammation, which may alleviate IBS symptoms.

Several small clinical trials and in vitro studies have evaluated the effects of L. salivarius (alone or in combination with other probiotics) on IBS. Some studies have reported improvements in specific symptoms, such as abdominal pain and bloating, and reduced inflammatory markers in patients who received L. salivarius compared to placebo. Notably, a double-blind, placebo-controlled study published in Alimentary Pharmacology & Therapeutics (2004) found that L. salivarius UCC4331 improved symptoms in patients with IBS. However, the number of well-designed, large-scale clinical trials specifically examining L. salivarius as a monotherapy for IBS is limited, and results are somewhat inconsistent across different studies.

Overall, there is scientific rationale and some preliminary supportive evidence for the use of L. salivarius in IBS, but the strength of this evidence is moderate to low (rated 2/5), and further high-quality research is needed to firmly establish its efficacy and optimal use in this condition.
