White Peony (Paeonia lactiflora) is primarily used in Traditional Chinese Medicine (TCM) for various gastrointestinal complaints, including symptoms that overlap with Irritable Bowel Syndrome (IBS) such as abdominal pain and spasms. In TCM, White Peony is thought to nourish the blood, regulate the liver, and alleviate pain, often being used in combination with other herbs (notably in the formula "Shao Yao Gan Cao Tang" or "Peony and Licorice Decoction") to relieve abdominal cramping and smooth muscle spasms. Historical texts and longstanding clinical experience in East Asia support its use for these symptoms, which align with some presentations of IBS. Scientific evidence, however, remains limited. Some preclinical studies and small clinical trials suggest that components of White Peony may have antispasmodic and anti-inflammatory effects on the gut, and that traditional formulas containing White Peony might reduce abdominal pain and frequency of diarrhea in IBS patients. However, these studies are generally low in quality, have small sample sizes, and often test multi-herb formulas rather than White Peony alone, making it difficult to attribute effects specifically to White Peony. As such, the evidence is best described as primarily traditional, with scientific support being preliminary and not robust enough to warrant a higher evidence rating.
