Slippery elm bark (Ulmus rubra) has a long history of use in North American traditional medicine, particularly among Indigenous peoples and early settlers. Its inner bark is known for its mucilaginous (gel-like) properties when mixed with water, which can soothe irritated tissues. Traditionally, slippery elm has been used to address a variety of mucosal and skin irritations, including sore throats, digestive discomfort, and topical wounds. For rectal itching (pruritus ani), the rationale for its use is primarily due to its demulcent action—when applied topically, slippery elm mucilage forms a protective barrier that can help reduce irritation and promote moisture retention on sensitive skin. Some traditional remedies involved making poultices or ointments with slippery elm bark to apply directly to inflamed or itchy areas. However, there is little to no direct scientific research specifically examining slippery elm's efficacy for rectal itching. Modern herbal references sometimes mention its use for soothing minor skin irritations or hemorrhoids, but these claims are extrapolated from its general demulcent and anti-inflammatory effects rather than robust clinical trials.
