Eicosapentaenoic acid (EPA) is an omega-3 polyunsaturated fatty acid commonly found in fish oil. There is scientific evidence supporting its use for joint health, particularly in the context of inflammatory joint diseases such as rheumatoid arthritis (RA). Multiple randomized controlled trials and meta-analyses have demonstrated that EPA, often in combination with docosahexaenoic acid (DHA), can reduce joint pain, morning stiffness, and the number of tender or swollen joints in patients with RA. The proposed mechanism involves EPA’s anti-inflammatory properties: EPA competes with arachidonic acid as a substrate for cyclooxygenase and lipoxygenase enzymes, leading to the production of less inflammatory eicosanoids and resolvins, which are involved in resolving inflammation. However, the evidence for EPA’s effectiveness in supporting joint health in osteoarthritis (OA) or in people without inflammatory joint disease is less robust. While EPA may offer some symptomatic relief, the magnitude of benefit is generally moderate and may not be clinically significant for all individuals. Major organizations, such as the American College of Rheumatology, acknowledge that omega-3 supplementation can be an adjunctive therapy in RA, though it is not a replacement for disease-modifying treatments. In summary, EPA has a moderate evidence base for supporting joint health in inflammatory conditions, with its benefits most clearly demonstrated in rheumatoid arthritis rather than in general joint support or osteoarthritis.
