Premna species, particularly Premna serratifolia (also known as "Agnimantha" in Ayurveda), have a long history of use in traditional medicine systems such as Ayurveda and traditional Chinese medicine. In Ayurveda, Premna is considered to have diuretic, anti-inflammatory, and antimicrobial properties and is included in formulations intended to support urinary tract health and kidney function. It is often found in classical polyherbal preparations used for conditions described as "Mutrakrichra" (painful urination or urinary tract disorders), which may encompass what is now classified as urinary tract or kidney infections. The rationale is based on its reputed ability to clear heat and toxins, promote urine flow, and reduce inflammation.

While there are a few in vitro and animal studies indicating that extracts of Premna possess antimicrobial and anti-inflammatory activity, direct clinical evidence for its effectiveness in treating kidney infections in humans is lacking. The scientific literature contains only limited pharmacological studies, and none have specifically addressed its use for kidney infection (pyelonephritis) in controlled human trials. Thus, its use for kidney infection is primarily justified by traditional knowledge rather than robust scientific validation.
