Khella (Ammi visnaga) has a longstanding history of use in traditional medicine, particularly in Middle Eastern and Mediterranean regions, for urinary tract conditions including kidney stones. Historically, the seeds and extracts of Khella were used as a folk remedy to "relax" the urinary tract and facilitate the passage of stones, likely due to its spasmolytic (smooth muscle relaxing) properties attributed primarily to its constituent khellin and visnagin. Some early 20th-century and mid-century studies, mostly observational and small in scale, suggested that Khella preparations may help reduce ureteral spasms, potentially assisting in the passage of stones or easing renal colic. However, robust clinical trials are lacking, and only limited preclinical studies have investigated its mechanism, mainly focusing on smooth muscle relaxation and mild diuretic effects. Modern systematic reviews and meta-analyses are absent. Thus, while the traditional use of Khella for kidney stones is well-documented, scientific validation is limited and evidence quality is low (hence the rating of 2).
