Sodium citrate is scientifically validated for use in the prevention and management of certain types of kidney stones, particularly those composed of uric acid and calcium oxalate. Sodium citrate acts as an alkalinizing agent; it is metabolized in the body to bicarbonate, which raises urine pH. This alkalinization of urine increases the solubility of uric acid, reducing the risk of uric acid stone formation. Additionally, higher urinary citrate levels inhibit the crystallization of calcium salts, thereby reducing the formation of calcium stones. Several clinical guidelines, including those from the American Urological Association and the European Association of Urology, recommend alkali therapy (potassium citrate or sodium citrate) for patients with recurrent calcium stones and hypocitraturia, as well as uric acid stones. Randomized controlled trials and observational studies have demonstrated that citrate supplementation can decrease stone recurrence rates and improve urinary parameters associated with stone risk. However, potassium citrate is generally preferred over sodium citrate, especially in patients with hypertension or heart failure, due to sodium load concerns. In summary, sodium citrate has a well-established, evidence-based role in the prevention and adjunct treatment of specific kidney stone types, supported by both clinical research and guideline recommendations.
