Trisodium citrate is scientifically validated for use in the management of certain types of kidney stones, particularly those composed of uric acid or calcium oxalate. The rationale for its use is based on its ability to alkalinize the urine and increase urinary citrate levels. Citrate is an important inhibitor of stone formation; it binds to calcium, reducing the availability of free calcium ions that can form insoluble stones. Alkalinization of urine further helps dissolve uric acid stones and prevents their formation. Clinical guidelines, including those from the American Urological Association, recommend the use of citrate salts (potassium citrate is most common, but sodium citrate is also used when potassium is contraindicated) for patients with recurrent calcium stones who have hypocitraturia, as well as for uric acid stones. Several randomized controlled trials and observational studies have demonstrated that citrate therapy reduces stone recurrence rates and increases urinary citrate excretion. While potassium citrate is generally preferred due to concerns about sodium load (which can increase urinary calcium), trisodium citrate is an effective alternative, especially in those who cannot tolerate potassium. Overall, the use of trisodium citrate in the prevention and management of kidney stones is supported by a strong scientific evidence base.
