Vitamin B6 (pyridoxal 5'-phosphate) has been traditionally used as a supportive agent in kidney stone prevention, particularly for calcium oxalate stones. The rationale behind its use stems from early studies in the mid-20th century, which suggested that vitamin B6 deficiency might increase urinary oxalate excretion—a risk factor for stone formation. Some small, older clinical trials and case reports indicated that vitamin B6 supplementation could reduce urinary oxalate levels, especially in individuals with primary hyperoxaluria (a rare genetic disorder that causes oxalate overproduction).
However, more recent and larger studies have provided inconsistent or minimal support for its effectiveness in preventing kidney stones in the general population. The majority of evidence supporting vitamin B6’s role comes from historical use and small-scale studies, rather than from large, well-controlled clinical trials. The American Urological Association guidelines do not currently recommend vitamin B6 supplementation for most patients with kidney stones, except in special cases like primary hyperoxaluria. While the theoretical mechanism—reducing endogenous oxalate production—remains plausible, scientific validation in broader patient populations is lacking.
