N-acetylcysteine (NAC) is used in certain clinical settings to support the kidneys, primarily to prevent contrast-induced nephropathy (CIN), a form of acute kidney injury that can occur after the administration of contrast agents during imaging procedures. NAC’s proposed mechanism is related to its antioxidant properties and its ability to replenish glutathione, a critical intracellular antioxidant. Early studies suggested that oral or intravenous NAC could reduce the incidence of CIN by scavenging free radicals and improving renal blood flow.

However, the scientific evidence is mixed. While several randomized controlled trials and meta-analyses have reported a modest benefit of NAC in reducing CIN risk, especially in high-risk patients (e.g., those with chronic kidney disease), other large, well-designed studies have found no significant effect compared to placebo. For example, a 2017 multicenter, randomized trial published in the New England Journal of Medicine found no significant difference between NAC and placebo in preventing kidney injury after contrast exposure.

Despite the inconsistent findings, NAC continues to be used in some protocols due to its low cost, favorable safety profile, and the lack of effective alternatives. Its use is largely restricted to the context of contrast-induced nephropathy prevention and not for general kidney “support.” There is no strong evidence for NAC supporting overall kidney health outside of this specific indication.
