Galactooligosaccharides (GOS) are prebiotic fibers that have been explored for their potential to support individuals with lactose intolerance. The scientific basis for their use is that GOS selectively stimulate the growth of beneficial gut bacteria, particularly Bifidobacteria, which can possess β-galactosidase (lactase) activity. This could potentially improve lactose digestion by enhancing the body’s ability to break down lactose in the colon. Several small clinical studies and in vitro experiments have demonstrated that GOS supplementation can increase populations of lactose-fermenting bacteria in the gut, leading to improvements in symptoms such as bloating and diarrhea in some lactose-intolerant subjects (Azcarate-Peril et al., 2013; He et al., 2008). However, the clinical evidence remains limited and not all studies have shown clear symptom relief, with some trials reporting only modest effects. Guidelines from gastrointestinal societies do not currently recommend GOS as a standard treatment for lactose intolerance, and more robust, large-scale clinical trials are needed. Overall, while there is a plausible scientific rationale and some early evidence, the strength of the evidence is moderate to low at this time.
