Benzalkonium chloride is a quaternary ammonium compound widely used as a topical antiseptic and disinfectant. Its use in treating or supporting lesions—specifically minor skin wounds, ulcers, or mucosal injuries—is primarily based on its proven antimicrobial properties. Benzalkonium chloride acts by disrupting microbial cell membranes, thereby reducing the risk of infection in minor skin breaks. Several in vitro and in vivo studies have demonstrated its effectiveness against a broad spectrum of bacteria, fungi, and some viruses.

In clinical practice, benzalkonium chloride is found in many over-the-counter antiseptic solutions, creams, and wipes intended for first aid of minor cuts, scrapes, and abrasions. However, its efficacy in promoting wound healing or preventing infection is considered moderate and somewhat inferior compared to other antiseptics, such as povidone-iodine or chlorhexidine. Some studies have raised concerns about its potential cytotoxicity to keratinocytes and fibroblasts, which may delay healing if used excessively or on large wounds.

Guidelines generally support its use for minor, superficial lesions (e.g., small cuts or abrasions) but recommend caution or alternative agents for deep, large, or chronic wounds. Overall, the use of benzalkonium chloride for lesion care is scientifically validated for infection prevention in minor wounds, but evidence for enhanced healing or superiority over other agents is limited.
