Sulphur has been used for centuries in traditional medicine, particularly in dermatology, to manage various skin conditions characterized by lesions, such as acne, scabies, seborrheic dermatitis, and fungal infections. Its use is documented in historical texts and pharmacopoeias as both a topical and oral remedy. The rationale behind its application is primarily its keratolytic (exfoliating) and antimicrobial properties, which help to remove dead skin cells and reduce the presence of bacteria or fungi on the skin. Modern clinical data supporting sulphur's efficacy for cutaneous lesions is limited, but some studies and case reports have indicated benefit in conditions like scabies and acne. For example, sulphur ointment (typically 5-10%) remains a recommended treatment for scabies in certain guidelines, especially when other agents are contraindicated. Adverse effects are generally mild, most commonly local irritation or dryness. While there is a lack of robust, high-quality randomized controlled trials, the long-standing traditional use and some supporting clinical observations justify its ongoing presence in dermatological practice for lesion treatment, though it is not considered first-line for most conditions today.
