Barleria species, particularly Barleria prionitis, are used in traditional medicine systems such as Ayurveda and folk medicine in parts of Asia and Africa for various ailments, including malaria. Ethnobotanical surveys report the use of Barleria leaves, roots, or whole plant extracts in decoctions or infusions as a supportive remedy for fever management associated with malaria. The traditional rationale is often based on the plant's perceived antipyretic (fever-reducing) and anti-inflammatory effects rather than direct antimalarial activity.

Scientific studies directly evaluating Barleria for antimalarial efficacy are limited. Some in vitro studies have indicated mild to moderate inhibitory effects against Plasmodium falciparum, the parasite responsible for the most severe form of malaria, but these findings are preliminary and do not match the efficacy of established antimalarial drugs. Most of the evidence remains anecdotal or based on ethnomedicinal documentation rather than robust clinical trials or pharmacological validation. Therefore, while Barleria is traditionally used for malaria-related symptoms, its direct antimalarial activity is not scientifically validated, and the overall evidence supporting its use is weak (rated 2/5).
