Eurycoma longifolia, commonly known as Tongkat Ali, is traditionally used in Southeast Asia as an aphrodisiac and remedy for male reproductive health, including enhancement of libido and management of infertility. In recent years, scientific studies have begun to investigate these traditional claims. Clinical trials indicate that supplementation with Eurycoma longifolia extract may improve certain aspects of male reproductive health, particularly testosterone levels and sperm quality. Some randomized controlled trials (RCTs) and meta-analyses have found moderate increases in serum testosterone and improvements in erectile function, sexual well-being, and sperm motility in subfertile men. The proposed mechanisms include stimulation of endogenous testosterone production and reduction in sex hormone-binding globulin (SHBG), leading to increased free testosterone. However, while the evidence is promising, it is not uniformly robust. Many studies are limited by small sample sizes, short durations, or potential conflicts of interest. Larger, well-designed RCTs are needed to confirm the efficacy and safety profile of Eurycoma longifolia. Nonetheless, the current body of evidence provides moderate scientific support for its use in supporting the male reproductive system, exceeding what would be expected from traditional use alone.
