Folic acid (Vitamin B9) plays an important role in DNA synthesis, cell division, and the regulation of homocysteine levels — all of which are essential processes for healthy sperm production and male fertility. Scientific studies have explored the association between folate status and various aspects of male reproductive health, including sperm count, motility, and morphology. Several observational studies have shown that men with higher folate intake or blood levels of folate tend to have improved semen quality. Furthermore, some clinical trials have found that supplementation with folic acid, often in combination with zinc, can lead to modest improvements in sperm concentration and motility in subfertile men.
However, the evidence is not universally consistent. Some randomized controlled trials have failed to show significant improvements in pregnancy rates or sperm parameters with folic acid supplementation alone. Nonetheless, folate deficiency is clearly associated with impaired spermatogenesis and increased sperm DNA damage, supporting its biological importance. Major health organizations do not universally recommend folic acid supplementation for all men, but it is sometimes included in male fertility supplements based on this supportive, though not conclusive, evidence base.
