Caprylic acid (octanoic acid) is a medium-chain triglyceride (MCT) found in coconut oil and palm kernel oil. Its use for supporting memory and brain function is primarily based on its role as an MCT, which can be rapidly metabolized by the liver into ketone bodies. These ketones can cross the blood-brain barrier and serve as an alternative energy source for neurons, especially in states where glucose utilization is impaired, such as in Alzheimer’s disease. Some studies have shown that MCT supplementation, which includes caprylic acid, may modestly improve cognitive function in patients with mild to moderate Alzheimer’s disease, particularly in those without the APOE4 genotype. One such product, caprylidene (Axona), was developed as a medical food for Alzheimer’s based on this mechanism, though clinical results have been mixed and improvements are generally small. The overall scientific support for caprylic acid specifically (as opposed to MCTs more broadly) is limited, and most studies are small or have methodological limitations. There is little to no traditional use of caprylic acid for cognitive health, and the scientific evidence, while promising, is not robust or conclusive. Thus, the use of caprylic acid for memory and brain function is justified by emerging scientific rationale but the quality and amount of evidence currently merit a moderate-low rating.
