Immunoglobulin G (IgG), particularly in the form of intravenous immunoglobulin (IVIG), has a scientifically validated role in specific cases of meningitis, though it is not a standard primary treatment for all forms of the disease. The mainstay therapy for bacterial meningitis remains targeted antibiotics, and for viral causes, supportive care or antiviral therapy where appropriate. However, IVIG has been used as an adjunctive therapy in certain situations—most notably in cases of meningitis caused by specific pathogens (such as enteroviruses in immunocompromised patients), in recurrent or chronic meningitis, and in patients with immunodeficiencies (e.g., X-linked agammaglobulinemia) who are at increased risk for severe or atypical infections. The rationale for IVIG use is its ability to provide passive immunity, modulate immune responses, and neutralize pathogens or their toxins. There are case reports and small studies suggesting that IVIG can be beneficial in viral meningitis, autoimmune-mediated forms (such as paraneoplastic or autoimmune encephalitis with meningeal involvement), and in certain bacterial infections where the host is unable to mount an adequate antibody response. However, large-scale randomized controlled trials are lacking, and its use is typically reserved for refractory, severe, or complicated cases rather than routine treatment. In summary, while IVIG (Immunoglobulin G) is not a frontline or universal treatment for meningitis, there is scientific precedent and clinical evidence supporting its use in selected clinical scenarios, particularly where immunodeficiency or immune-mediated pathology is present.
