Vitamin D has garnered scientific interest for its potential role in menstrual regularity, particularly in women with polycystic ovary syndrome (PCOS) and other reproductive disorders. Several studies have found that women with menstrual irregularities, especially those with PCOS, often have lower serum vitamin D levels compared to women with regular cycles. Vitamin D is thought to influence ovarian follicular development and regulate menstrual cycles by modulating anti-Müllerian hormone (AMH) levels and improving insulin sensitivity.

Randomized controlled trials and observational studies have examined the effect of vitamin D supplementation on menstrual patterns. Some small studies have shown modest improvements in menstrual frequency and ovulation rates following vitamin D supplementation, particularly in women who were deficient at baseline. However, results are inconsistent, and many studies are limited by small sample sizes or confounding factors such as concurrent use of other supplements (e.g., calcium, metformin).

Major guidelines do not currently recommend vitamin D as a standalone treatment for menstrual irregularity but do support correcting vitamin D deficiency for overall health. While the biological plausibility exists and preliminary evidence is promising, more robust, large-scale clinical trials are needed to confirm efficacy. Thus, current scientific evidence is limited (rated 2/5), and vitamin D should not be considered a primary therapy for menstrual irregularity except in cases of deficiency or as part of a broader treatment strategy.
