12-Methylcarnosic acid is a diterpene compound derived from rosemary (Rosmarinus officinalis). While it is not as extensively studied as carnosic acid itself, there is emerging scientific interest in its potential metabolic benefits. The rationale for its use in metabolic syndrome is primarily based on preclinical research demonstrating antioxidant, anti-inflammatory, and lipid-lowering properties. One notable study (Kim et al., 2018) found that 12-methylcarnosic acid could improve insulin resistance and reduce hepatic lipid accumulation in high-fat diet-induced obese mice. The mechanisms proposed include activation of AMP-activated protein kinase (AMPK) and suppression of inflammatory pathways such as NF-κB, both relevant to the pathophysiology of metabolic syndrome. However, human clinical trials specifically evaluating 12-methylcarnosic acid for metabolic syndrome are lacking. Most of the available evidence comes from in vitro assays or animal models, and results from such studies do not always translate to similar efficacy in humans. Thus, while there is preliminary scientific validation for its use in metabolic syndrome, the evidence is limited and not yet robust.
