7-hydroxymatairesinol (HMR) is a plant lignan primarily found in Norway spruce (Picea abies) and is metabolized in the body to enterolignans, which have been studied for their potential health benefits, including effects on metabolic syndrome. The scientific rationale for using HMR to support or treat metabolic syndrome is based on preclinical (animal and cell line) studies and a limited number of early-phase human trials. These studies suggest that HMR and its metabolites may have beneficial effects on lipid metabolism, glucose homeostasis, and inflammation—all of which are relevant to metabolic syndrome, a cluster of conditions that includes obesity, dyslipidemia, insulin resistance, and hypertension.

Specific mechanisms proposed include the modulation of PPARγ activity, reduction of oxidative stress, and improvement in insulin sensitivity. Animal studies have shown that HMR supplementation can reduce body weight gain, improve glucose tolerance, and lower plasma cholesterol and triglyceride levels. Human data, however, are sparse, and the few small-scale clinical trials conducted to date have produced mixed results, with some indicating modest improvements in cholesterol profiles, but insufficient evidence to confirm a clear benefit for metabolic syndrome as a whole.

Overall, while the mechanistic data and animal studies provide a basis for further exploration, robust clinical evidence is lacking. Therefore, HMR’s use for metabolic syndrome is primarily supported by preclinical scientific findings rather than established clinical efficacy.
