Anthocyanidins are a class of flavonoids found in various fruits and vegetables, particularly those with red, purple, or blue pigmentation (such as berries, grapes, and red cabbage). Scientific interest in anthocyanidins and anthocyanins (their glycosylated forms) stems from their antioxidant, anti-inflammatory, and vasoprotective properties. Several preclinical studies and some small clinical trials have explored their potential benefits in metabolic syndrome—a cluster of conditions that includes insulin resistance, hypertension, dyslipidemia, and central obesity.

Evidence from animal studies suggests that anthocyanidins can improve insulin sensitivity, reduce blood pressure, and ameliorate lipid profiles. Some randomized controlled trials in humans, using anthocyanin-rich extracts or foods (such as blueberry or bilberry supplements), have shown modest improvements in markers like fasting blood glucose, lipid profiles, and blood pressure in populations at risk for metabolic syndrome. However, these effects are generally mild and not always consistent across studies.

Systematic reviews and meta-analyses indicate a potential benefit, but limitations include heterogeneity in study design, anthocyanidin source, dosage, and duration. Overall, while there is some scientific evidence to support the use of anthocyanidins in metabolic syndrome, the strength of the evidence is currently low to moderate and further well-designed clinical trials are needed before definitive conclusions can be drawn.
