Lipstatin is a natural product isolated from Streptomyces toxytricini, and is best known as the precursor molecule for the anti-obesity drug orlistat (tetrahydrolipstatin). Orlistat is a well-established inhibitor of pancreatic and gastric lipases, enzymes involved in the breakdown and absorption of dietary fats. By inhibiting these enzymes, orlistat reduces the absorption of triglycerides from the diet, leading to decreased caloric intake and weight loss. Multiple clinical trials have demonstrated that orlistat can produce modest but significant weight loss in overweight and obese individuals, and it has also been shown to improve some components of Metabolic Syndrome, particularly by reducing waist circumference, improving lipid profiles (such as lowering LDL cholesterol and total cholesterol), and in some cases improving glycemic control.

Although pure lipstatin itself is not used clinically due to its instability, its derivative orlistat is approved for the treatment of obesity, which is a major risk factor and component of Metabolic Syndrome. The scientific evidence supporting orlistat’s use in improving metabolic parameters relevant to Metabolic Syndrome is substantial, though its effects are generally modest and most pronounced when combined with lifestyle interventions. The evidence rating is not higher than 3 because while there are improvements in Metabolic Syndrome markers, orlistat (and by extension, lipstatin) is not a primary therapy for Metabolic Syndrome as a whole, but rather for its obesity component.
