Nicotinamide riboside (NR) is a form of vitamin B3 and a precursor to nicotinamide adenine dinucleotide (NAD+), an essential molecule involved in cellular metabolism and energy production. The scientific rationale for using NR to support or treat Metabolic Syndrome (MetS)—a cluster of conditions including insulin resistance, obesity, hypertension, and dyslipidemia—stems from preclinical studies showing that increasing NAD+ levels can improve mitochondrial function, enhance energy metabolism, and reduce inflammation. Several animal studies have demonstrated that NR supplementation can improve glucose tolerance, insulin sensitivity, and lipid profiles, and may even reduce weight gain in high-fat diet models.

However, human clinical evidence remains limited and somewhat inconsistent. A handful of small, short-term randomized controlled trials in humans have investigated NR supplementation (typically 1–2 g/day) in overweight or obese individuals, some with features of MetS. While NR effectively raises NAD+ levels in blood and tissues, the impact on metabolic outcomes has been modest. Some studies report slight improvements in hepatic fat content or inflammatory markers, but most trials report no significant changes in insulin sensitivity, glucose metabolism, blood pressure, or body weight. No major safety concerns have been observed at the studied doses.

In summary, there is a plausible mechanistic basis and encouraging preclinical evidence for NR in Metabolic Syndrome, but human clinical evidence is preliminary, with limited efficacy demonstrated so far. Thus, the scientific support is present but weak, warranting a moderate-low evidence score.
