Petasin is a key active compound found in Petasites hybridus (butterbur), a plant traditionally used in Europe for various ailments. Its use for migraine prevention is supported by several clinical studies and has been recognized in some medical guidelines. The primary mechanism believed to underlie its effect is the inhibition of leukotriene synthesis and reduction of smooth muscle spasms, both of which may be involved in migraine pathophysiology.

Two randomized, double-blind, placebo-controlled trials (Lipton et al., 2004; Diener et al., 2004) demonstrated that standardized butterbur extract (containing petasin and isopetasin) significantly reduced migraine attack frequency compared to placebo. The American Academy of Neurology (AAN) and the American Headache Society (AHS) in 2012 gave butterbur a Level A recommendation for migraine prevention, although concerns about liver toxicity and product quality have since led some organizations to withdraw or revise this recommendation.

While the evidence is reasonably strong (hence a score of 3), it is tempered by safety concerns, particularly regarding pyrrolizidine alkaloids (PAs) that can cause hepatotoxicity if not properly removed from extracts. As a result, only PA-free, standardized butterbur extracts are considered for use. In summary, there is scientific evidence supporting the use of petasin-containing butterbur extracts for migraine prophylaxis, but safety and standardization are significant considerations.
