Vitamin D has received attention in recent years for its potential role in supporting pregnancy and preventing miscarriage, based on emerging scientific evidence. Several observational studies have noted that vitamin D deficiency is relatively common among pregnant women and is associated with an increased risk of various adverse pregnancy outcomes, including miscarriage, preeclampsia, and preterm birth. Vitamin D is thought to support immune modulation, implantation, and placental function, all of which are critical processes for maintaining a healthy pregnancy.

However, while low vitamin D levels are correlated with higher miscarriage rates, intervention studies providing supplementation have produced mixed and generally inconclusive results. A 2017 systematic review and meta-analysis published in Fertility and Sterility found a possible association between sufficient vitamin D status and reduced risk of pregnancy loss, but emphasized that the evidence was not robust enough to recommend supplementation specifically for miscarriage prevention. Most guidelines recommend vitamin D supplementation during pregnancy primarily for maternal bone health, rather than specifically to prevent miscarriage.

In summary, vitamin D is supported by some scientific evidence as potentially beneficial in reducing miscarriage risk, but high-quality randomized controlled trials are lacking, and causality is not firmly established. Thus, while there is a plausible biological rationale and some observational support, recommendations for vitamin D specifically to prevent miscarriage should be made cautiously.
