Alpha-lipoic acid (ALA) is a naturally occurring compound that acts as a cofactor for mitochondrial enzyme complexes, particularly those involved in energy metabolism such as the pyruvate dehydrogenase and alpha-ketoglutarate dehydrogenase complexes. Its scientific use for mitochondrial support is based on both its direct role in mitochondrial energy production and its potent antioxidant properties. ALA can scavenge reactive oxygen species and regenerate other antioxidants, such as vitamins C and E, thereby protecting mitochondrial components from oxidative damage.
Multiple preclinical studies have demonstrated that ALA supplementation improves mitochondrial function, increases ATP production, and reduces oxidative stress in various tissues. In animal models of mitochondrial dysfunction and aging, ALA has been shown to restore mitochondrial enzyme activities and enhance mitochondrial biogenesis. Human clinical studies, though fewer, suggest potential benefits in conditions characterized by mitochondrial impairment, such as diabetic neuropathy and some neurodegenerative diseases. However, while mechanistic and animal data are robust, large-scale clinical trials directly targeting mitochondrial disorders in humans are limited.
Overall, the evidence supporting the use of ALA for mitochondrial support is substantial from a mechanistic and preclinical standpoint, with emerging but still limited clinical validation. It is widely used in integrative medicine for this purpose, supported by its biochemical role and antioxidant effects.
