Vitamin B12, specifically in its coenzyme form adenosylcobalamin, plays a scientifically validated role in mitochondrial function. Adenosylcobalamin serves as a cofactor for the mitochondrial enzyme methylmalonyl-CoA mutase, which is crucial for the metabolism of certain amino acids and fatty acids. This enzyme catalyzes the conversion of methylmalonyl-CoA to succinyl-CoA, an important intermediate in the Krebs cycle—a central pathway for energy production in mitochondria.

Deficiency of vitamin B12 results in the accumulation of methylmalonic acid, which can impair mitochondrial energy production and cause neurological symptoms. Clinical studies have shown that B12 deficiency can lead to mitochondrial dysfunction, manifesting as muscle weakness, fatigue, and neurodegenerative changes. Supplementation with B12 restores normal mitochondrial function in deficient individuals.

While most research relates to the prevention and correction of deficiency, rather than direct enhancement of mitochondrial performance in healthy individuals, the essential role of adenosylcobalamin in mitochondrial metabolism is well established by biochemical and clinical evidence. Therefore, the use of vitamin B12 to support mitochondrial health is scientifically justified, particularly in individuals with or at risk of deficiency.
