MK-677 (also known as Ibutamoren) is a non-peptide growth hormone secretagogue that stimulates the secretion of growth hormone (GH) and insulin-like growth factor 1 (IGF-1). Its use in the context of muscular dystrophy is based on the hypothesis that increased GH and IGF-1 levels could counteract muscle wasting and promote muscle growth. Some preclinical studies and early-phase clinical trials have examined MK-677 in populations with muscle-wasting conditions, including older adults and individuals with GH deficiency. These studies have shown increased lean body mass and, in some cases, modest improvements in muscle strength or function. However, specific scientific evidence supporting the use of MK-677 directly for muscular dystrophy is limited. A small number of clinical trials (e.g., a 2018 Phase 2 study in Duchenne Muscular Dystrophy) have explored MK-677 for this indication, but results are preliminary and not conclusive. The trials suggest potential for increased muscle mass but have not demonstrated clear improvements in functional outcomes. Furthermore, there are concerns regarding safety, including potential effects on glucose metabolism and long-term risks. In summary, while there is a scientific rationale and some early evidence supporting the investigation of MK-677 for muscular dystrophy, robust, high-quality clinical evidence is lacking. Therefore, its use is not currently standard or widely recommended for this condition.
