Vitamin B6, also known as pyridoxine (not pyridoxamine HCl), has scientific validation for use in the management of nausea and vomiting, particularly in pregnancy (NVP). Multiple randomized controlled trials and meta-analyses have demonstrated that vitamin B6 supplementation is effective in reducing the severity of nausea and frequency of vomiting in pregnant women. It is often recommended as a first-line therapy in guidelines for the management of mild to moderate NVP. The typical doses studied are 10–25 mg taken three times daily. Major professional organizations, such as the American College of Obstetricians and Gynecologists (ACOG), endorse vitamin B6 as a safe and effective intervention for NVP. The mechanism of action is not fully understood, but it is thought to involve the role of vitamin B6 in neurotransmitter synthesis, which may influence the central nausea center. It is important to note, however, that while pyridoxine (vitamin B6) is the form used in clinical studies and practice, pyridoxamine is a different form of vitamin B6 and is not typically used or studied for this indication. There is limited evidence for the use of pyridoxamine HCl specifically for nausea and vomiting. Therefore, the scientific evidence rating of 4 applies to pyridoxine HCl, not pyridoxamine HCl. Use of vitamin B6 for other causes of nausea outside pregnancy is less well established.
