Astragaloside, a key active component of Astragalus membranaceus, has been investigated for its potential benefit in nephritis, particularly in the context of chronic kidney disease (CKD) and glomerulonephritis. Preclinical studies, mainly in animal models and in vitro experiments, have shown that astragaloside IV may reduce proteinuria, inhibit renal fibrosis, attenuate inflammatory responses, and mitigate oxidative stress—mechanisms involved in the pathogenesis of nephritis. Some studies suggest that astragaloside IV can modulate immune function and protect podocytes, which are often damaged in nephritis. However, robust clinical trials in humans are lacking. Most human evidence comes from small-scale studies or is extrapolated from the broader use of Astragalus-based formulas in traditional Chinese medicine for kidney health.

Systematic reviews and meta-analyses of Astragalus preparations (not isolated astragalosides) have shown modest benefit in improving renal function markers, but the quality of evidence is generally low due to methodological issues, heterogeneity of interventions, and small sample sizes. Therefore, while there is a scientific rationale and some experimental evidence for using astragaloside in nephritis, clinical validation remains limited. The current evidence level is rated as 2 out of 5, indicating preclinical promise but insufficient high-quality human data to fully support its use for nephritis.
