The rationale for its use stems from its ability to cross the blood-brain barrier more effectively than standard thiamine, potentially enhancing central nervous system thiamine levels.
Some small-scale clinical studies and user reports suggest sulbutiamine may improve asthenia (physical and mental fatigue), particularly in the context of infectious diseases or chronic fatigue syndromes. For example, clinical trials in the 1970s and 1980s in France reported improvements in fatigue with sulbutiamine compared to placebo. However, these studies were limited by small sample sizes, short durations, and methodological weaknesses.
There is no substantial evidence from large, high-quality randomized controlled trials to firmly support the use of sulbutiamine or vitamin B1 for "nervous exhaustion" as a distinct clinical entity. The term itself is not well-defined in modern medicine.
