Vitamin B12 (as adenosylcobalamin or other active forms) is scientifically recognized for its critical role in nervous system function. Deficiency in vitamin B12 can lead to a range of neurological symptoms, including fatigue, cognitive disturbances, peripheral neuropathy, and in severe cases, psychiatric manifestations. The term "nervous exhaustion" is somewhat archaic, but it often refers to symptoms of fatigue, mental weariness, and sometimes depressive symptoms, which can overlap with the clinical picture of B12 deficiency.

Several clinical studies and reviews have demonstrated that B12 supplementation can reverse neurological symptoms caused by B12 deficiency, particularly in individuals with pernicious anemia or malabsorption syndromes. Restoration of B12 levels often leads to improvement in energy, mood, and cognitive function when deficiency is present. However, evidence for the use of B12 in individuals with normal B12 levels (i.e., without deficiency) for "nervous exhaustion" or general fatigue is limited and inconsistent. Most benefits are observed specifically in those who are deficient.

Therefore, the scientific evidence supports the use of vitamin B12 to treat neurological symptoms secondary to deficiency, which can present as nervous exhaustion. There is insufficient evidence to recommend its use for nervous exhaustion in the absence of confirmed B12 deficiency.
