Nicotinamide (also known as niacinamide), a form of vitamin B3, has been traditionally used in some regions—especially in parts of Asia and Eastern Europe—to support or treat neuralgia and neuritis. This practice is rooted more in empirical or historical use than in robust modern scientific validation. The rationale stems from the established role of B vitamins in nerve health and the prevention of deficiency-related neuropathies, such as pellagra, which can cause neurological symptoms. Nicotinamide has been included in combination B-vitamin preparations for neuropathic pain and neural inflammation, particularly when deficiency is suspected. However, in cases where neuralgia or neuritis is not due to vitamin B3 deficiency, the evidence for nicotinamide’s effectiveness is limited. A small number of older clinical studies and case reports suggest potential benefits of high-dose B vitamins, including nicotinamide, in certain neuropathic syndromes, but these are generally of low methodological quality and not specific to nicotinamide alone. Recent reviews and clinical guidelines do not recommend nicotinamide as a first-line or evidence-based therapy for neuralgia or neuritis outside of deficiency states. Therefore, while its use is supported by tradition and some plausible rationale, strong scientific validation is lacking, and the overall quality of evidence is low (rated 2/5).
