Paeoniflorin is a major bioactive constituent of the root of Paeonia lactiflora, commonly used in traditional Chinese medicine. Its use for neurosis—a term historically referring to various non-psychotic mental disorders, such as anxiety, depression, and somatoform disorders—has some scientific basis, particularly from preclinical studies. Several animal studies have demonstrated that paeoniflorin exhibits anxiolytic and antidepressant-like effects. These effects are believed to be mediated via modulation of neurotransmitter systems, including serotonin, dopamine, and GABAergic signaling. Additionally, paeoniflorin has been shown to exert neuroprotective effects, reduce neuroinflammation, and modulate the hypothalamic-pituitary-adrenal (HPA) axis, all of which are implicated in the pathophysiology of neurosis-related conditions.

However, the evidence is primarily limited to in vitro experiments and animal models. Clinical trials in humans specifically addressing neurosis or related psychiatric conditions are scarce and generally of low methodological quality. Most human data comes from studies of traditional Chinese herbal formulas containing paeoniflorin as one component, making it difficult to attribute effects specifically to paeoniflorin. As such, while there is a scientific rationale and some supporting data for its use in neurosis, the current evidence base remains preliminary and insufficient to robustly support its clinical application for this indication.
