Acemannan is a polysaccharide extracted from the inner leaf gel of Aloe vera and has been investigated for its potential benefits in oral surgery and wound healing. Several in vitro and in vivo studies have demonstrated acemannan's ability to promote fibroblast proliferation, collagen synthesis, and angiogenesis, all of which are vital for oral tissue regeneration. Notably, randomized controlled trials have shown that the topical application of acemannan hydrogel can enhance the healing of oral ulcers and extraction sockets, with improvements in epithelialization rates and reduced healing time compared to controls. For example, a 2012 randomized controlled trial by Choonhakarn et al. found that acemannan hydrogel significantly accelerated healing of aphthous stomatitis. Additionally, studies like those by Kaur et al. (2019) and Yuniartha et al. (2017) have reported improved post-extraction socket healing and bone formation with acemannan application in animal models.

However, while results are promising, most clinical trials are small and some lack long-term follow-up. Larger, high-quality studies are needed to confirm these effects and determine optimal dosing and delivery. Nonetheless, current evidence supports the scientific rationale for acemannan's use as an adjunct in oral surgery for enhancing tissue repair and reducing recovery time.
