Lactoferrin is a multifunctional glycoprotein found in various secretory fluids, including saliva, and plays a critical role in the innate immune response. Its use in oral surgery is supported by emerging scientific evidence, primarily due to its antimicrobial, anti-inflammatory, and wound-healing properties. Several in vitro and animal studies have demonstrated that lactoferrin can inhibit the growth of oral pathogens, including bacteria associated with periodontal disease and postoperative infections. In addition, lactoferrin modulates the inflammatory response and promotes tissue regeneration, both of which are crucial for optimal healing following oral surgical procedures such as extractions or periodontal surgery.

Clinical studies are still relatively limited but promising. Some randomized controlled trials and pilot studies suggest that lactoferrin-containing oral gels or mouthwashes can reduce postoperative inflammation, accelerate wound healing, and lower the risk of surgical site infections. For example, topical application of lactoferrin has been shown to enhance the healing of extraction sockets and improve outcomes in patients undergoing periodontal therapy.

While more large-scale human trials are needed to definitively establish dosing and long-term benefits, the current body of evidence supports the adjunctive use of lactoferrin to improve oral surgical outcomes. Thus, its use is justified by scientific validation, though the level of evidence is moderate and continuing to evolve.
