Diosgenin is a naturally occurring steroidal saponin found primarily in plants such as wild yam (Dioscorea spp.). Its use in supporting or treating osteoporosis is based on its structural similarity to estrogen, a hormone that plays a critical role in bone health. Several in vitro and animal studies have indicated that diosgenin may have beneficial effects on bone metabolism. For example, research in ovariectomized rats (an animal model for postmenopausal osteoporosis) has shown that diosgenin can increase bone mineral density and enhance bone formation while reducing bone resorption. These effects are believed to be related to modulation of estrogen receptors and various signaling pathways involved in bone remodeling, such as the OPG/RANKL pathway.

Despite these promising preclinical findings, there is a lack of robust clinical evidence in humans to confirm its efficacy and safety in treating or preventing osteoporosis. Most human data are lacking, and diosgenin is not an approved or widely recognized treatment for osteoporosis in medical guidelines. While the scientific rationale exists and preliminary evidence appears positive, more high-quality clinical trials are needed before diosgenin can be recommended as a therapeutic agent for osteoporosis in humans.
